I agree for me or my child/ward to attend the venue and to undertake all activities and/or participate in the registered program or activity. 

If you are aware of any personal health problems your child/ward is experiencing, we urge you to see your doctor before participating in any of our programs and activities.

Acknowledgment and Assumption of Risk

I acknowledge that activities organised or operated by Urban Rec t/a Jamboree Sports may involve risks connected with any form of physical activity including activities usually undertaken in team sports and agree to accept all such risks and dangers. 

In the case of an emergency, I authorise Urban Rec t/a Jamboree Sports, and associated staff, where it is impracticable to communicate with me, to arrange for my child/ward to receive such medical or surgical treatment as may be deemed necessary. 

I also undertake to pay or reimburse costs which may be incurred for medical attention, ambulance transport, and medication while my child/ward is attending the Centre/enrolled in the program. I understand that although Urban Rec t/a Jamboree Sports and its service providers attempt to minimize any risk of personal injury within practical boundaries, accidents do happen and all physical activities carry the risk of personal injury. I acknowledge that there is an inherent risk of personal injury in physical activities that will be undertaken at the Centre/as part of the program and I accept that risk on behalf of my child/ward. 

Waiver and Release:

I hereby agree to voluntarily release, discharge and fully indemnify Urban Rec t/a Jamboree Sports, its employees, agents, servants, representatives, and insurers against any injury or accident to myself, or loss or damage done to property in my care that arises out of, or in connection with, my involvement or participation in any activities organised or operated by Urban Rec t/a Jamboree Sports. I agree to abide by any instructions given by Urban Rec t/a Jamboree Sports, its employees, agents, servants, and representatives during my involvement or participation in activities organised or operated by Urban Rec t/a Jamboree Sports.

Permission to use images, video, and voice of participants:

Urban Rec t/a Jamboree Sports develops promotional materials and publishes them in print and digitally (eg on websites). By acknowledging this waiver and registering your child/ward, you grant permission for Urban Rec t/a Jamboree Sports to create/use photographs, video, or audio recordings of you or your child/ward and to distribute them in printed publications (eg newsletters, promotional material) and publicly accessible websites, including social media accounts. The permission will continue until you revoke permission in writing to us.
